Our sales department receives numerous orders each day. Due to high fraudulent activity, we have implemented a thorough order verification process for flagged orders.

This helps us verify that the person creating and placing the order matches the billing information provided. In order for us to process your order, we need to verify your identity due to suspicious information flagged on your account, such as incorrect billing or shipping information, mismatched card information, high risk of chargeback, unauthorized use, or similar concerns.

Failure to submit this form within 1 hour after receiving email notice may result in your order being cancelled immediately.
